Documents Required for Name Change In Affidavit

Minors under the age of 18 (Mandatory Documents)

• Aadhar card, school id are acceptable forms of identification.
• Ration card or Aadhar card as proof of address
• Birth certificates or bonafide certificate from school or college are both acceptable forms of proof of age.
• Two passport-sized photograph of minor
• Two passport-sized photograph of their parents of guardian
• If possible, the minor's signature and that of his or her parents or guardians must be included on the application form
Adults and Majors (Mandatory Documents )
• Pan Card, Passport, Driver's License, Aadhar Card, or Voters Id are all acceptable forms of identification
• Ration card, Aadhar card, or utility bill as evidence of address
• One passport-sized photograph
• Form of Application: Completed and signed application form with all pertinent information Names, both old and new
• Address for correspondence, contact information, and the reason for the name change
• Marriage certificate (optional) ( if Any for Females after Marriage )
• The court has issued a divorce decree Court (in the event of a divorce, for example) The court issues an adoption deed ( In case of Adoption )
